Thirunanthikarai Cave Temple or Thirunandhikarai Cave Temple, situated in Thirparappu panchayath of Kanyakumari District, is a popular pilgrim destination. Thirunanthikarai Cave Temple is dedicated to Lord Shiva. It is considered as the fourth temple of Shivalyam.

Thirumalai, Thikkurichi, Thiruparappu, Thirunanthikkarai, Ponmanai, Pannippagam, Kalkulam, Melancode, Thiruvidaicode, Thiruvidamcode, Thiruppanticode and Thiru Nattalam are the twelve temples of Shivalyam.

Thirunanthikkarai Temple is situated on the river banks of Nandhiaaru. The temple is built in the traditional Kerala style of temple architecture. Though the temple is the abode of Shiva, a shrine of Lord Vishnu is also present in the temple facing Lord Shiva.

There is an ancient rock-cut cave temple situated towards the northern side of this temple. This ancient temple is believed to be constructed in between the 7th – 8th centuries by Jains. It remained a sacred place for Jains up to the late 9th century AD.

It was later seized by Raja Raja Chola in the 10th century AD and a lingam was installed in this temple. The stone inscriptions related to the birthday celebrations of Raja Raja Chola and the capture of Muttom can be found in this rock-cut temple.

The rock-cut cave temple of Thirunanthikkarai temple was a storehouse of many ancient sculptures among which many have come to ruins. The remnants of Thirunanthikkarai rock-cut cave temple are now under the care of the Archaeological Survey of India.
